The 5 questions for the course evaluation are the following :
1) Did the material and the content inform you sufficiently?
2) Were the structure and progress of the course adequate?
3) Has the teacher been clear and organized enough for your understanding?
4) Has the teacher answered the students' questions and been receptive?
5) Was the workload adequate (assignments, exams, readings)?

You can answer these 5 questions by using this scale of 1 to 5
with 1 = not really, 2 = sort of 3 = in general 4 = most certainly 5 = undoubtedly

example : Question 1) 3 ; Question 2) 4 ; Question 3) 2 ... etc.
